چکیده
Nos encontramos ante una de las oportunidades más importantes de la pediatría para mejorar la atención de las personas: neonatos, lactantes, niños y niñas, adolescentes1 y la de sus familias, incluso la nuestra, la de los profesionales. La razón de ser de la atención paliativa es optimizar la calidad de vida, cuando se padece una enfermedad grave e incurable que amenaza o limita la vida, centrándose en el paciente y su familia a través de una atención integral de sus diferentes dimensiones: física, psicosocial y espiritual1. Esta atención también se debe dar en aquellos casos en los que es probable que se produzca la muerte antes de nacer o en los que, en el momento del diagnóstico, sabemos que pueden vivir sólo horas o minutos. Afortunadamente en las últimas décadas ha habido un descenso de la mortalidad infantil en nuestro medio aunque esto ha supuesto un aumento de la morbilidad. En muchos casos una comorbilidad crónica, a la que en algunos casos llamamos ‘compleja’ tal vez para justificar la carencia de respuesta a las necesidades reales, la atención fragmentada que ofrecemos y el gasto por la falta de eficiencia de un sistema demasiado ‘hospital centrado’. En realidad la complejidad, representa aquellos casos en los que el ‘todo’ es diferente, y más, que la suma de todas sus partes y depende de las variabilidades individuales y también de la capacidad
